Answers about CVD risk assessment, remote care and using WeGuide in your practice.
A cardiovascular risk assessment, sometimes called a heart disease risk assessment, estimates the chance of a heart attack or stroke over a set period. It combines risk factors such as age, blood pressure, cholesterol, smoking and diabetes, many of which cause no symptoms. A heart health check includes this assessment and a conversation with a clinician about prevention and follow-up.
It usually includes questions about health, medicines, family history, smoking, alcohol and physical activity, together with a blood pressure check and a cholesterol test. Some checks also include a blood sugar test and weight and waist measurements. The clinician decides which measurements and tests are needed, reviews the findings and discusses a prevention plan.
Clinicians use a validated risk calculator or risk chart suited to the patient and to local guidance. It combines factors such as age, blood pressure, cholesterol, smoking and diabetes to estimate risk over a set period, often five or ten years. Calculators differ by country and region, and some can be used without a blood test. Clinical judgement remains essential when interpreting the result.
Adults without known heart disease are usually offered a check from middle age, and in some guidance earlier, for example with diabetes. Anyone concerned about high blood pressure, high cholesterol, smoking or a family history of heart disease can discuss a check with their clinician. Starting ages and repeat intervals differ by country. People who already have heart disease usually have regular reviews instead.
No. A cardiovascular risk assessment estimates the chance of a future heart attack or stroke from risk factors such as blood pressure, cholesterol and smoking. An ECG is a quick, painless test that records the heart's electrical activity, including its rate and rhythm. The two serve different purposes, and a clinician may arrange an ECG or other heart tests separately when appropriate.
Parts of it can. Health history and lifestyle questionnaires, home blood pressure readings with a validated device where the clinician agrees, education and follow-up can happen between visits, and some services offer parts of the check online. Blood tests and any physical examination or ECG need a local appointment. The care team decides which steps are suitable for remote completion.
Remote patient monitoring helps a care team collect agreed measurements and patient-reported updates between appointments. In a cardiovascular care pathway, these can support follow-up conversations and prevention goals. WeGuide brings scheduled questionnaires, reminders, patient education and check-ins into the digital journey configured by the care team, with readings entered by the patient in the app.
The clinician and patient agree on a prevention plan, which may include lifestyle goals, referral to support services, treatment discussions and repeat measurements. Because risk rises with age, the assessment is usually repeated over time. WeGuide supports this ongoing journey with patient guidance, scheduled check-ins and reminders, configured by the care team.
Contact WeGuide to discuss your cardiovascular care pathway, patient population and existing workflow. Together, we can identify the intake forms, patient education, follow-up schedule and any integrations needed to support your service. Your clinical team defines the assessment protocol, chooses and applies the risk calculator, and makes all care decisions.
WeGuide pricing depends on the scope of your care pathway, configuration, integrations and support requirements. Contact our team to discuss your service and request a proposal. Clinical consultation and testing fees are determined separately by the care provider, and WeGuide does not supply blood pressure devices or laboratory testing.
