Answers about cardiac rehabilitation, cardiac rehab at home and using WeGuide in your program.
Cardiac rehabilitation, or cardiac rehab, is a structured program of exercise, education and support for people recovering from a heart attack, heart surgery or a heart procedure, or living with heart failure or another heart condition. It is led by a team of health professionals and aims to help people return to an active life and lower the risk of further heart problems.
A cardiac rehab program usually starts with an assessment of your health, concerns and goals. It then combines exercise tailored by the team, education on topics such as healthy eating, medicines and stopping smoking, and support for stress, anxiety or low mood. Programs differ in length and format, and the team agrees a plan that suits each person.
Before the program, the team reviews your health and history and may arrange tests to check your heart. During exercise, effort is often judged by heart rate or a rating scale such as the Borg CR10. The team may also carry out walking tests such as the 6-minute walk test, and use the KCCQ for people with heart failure.
Cardiac rehab can help people who have had a heart attack, coronary angioplasty or a stent, heart or heart valve surgery, or a heart transplant, and people living with angina, heart failure or peripheral arterial disease. People of any age or fitness level can benefit. If it has not been mentioned after a heart event, you can ask your clinician whether a program is right for you.
No. Exercise is a central part, but cardiac rehabilitation is a supervised program built around your heart condition. It begins with an assessment, uses exercise tailored and monitored by health professionals, and adds education, lifestyle support and help with emotional wellbeing. A general exercise class does not include this clinical assessment or follow-up.
Yes, where it is offered. Home-based and virtual programs use phone, video, websites or apps, with health professionals supporting from a distance, and hybrid programs combine home and centre sessions. Reviews found home-based and centre-based programs similarly effective for people recovering from a heart attack or revascularisation, or living with heart failure. Assessments and some tests still need a visit, and the team decides what suits you.
With WeGuide, the cardiac rehab team configures readiness questions, education modules, reminders and an exercise log for the activity they prescribe, with check-ins on symptoms, mood and effort. Steps and heart rate can sync from Apple Health, Health Connect or Garmin where available. The team reviews progress as agreed. WeGuide is not for emergencies: chest pain or other urgent symptoms need care straight away through local emergency services.
After the program, the aim is to keep up regular activity and heart-healthy habits for the long term, with follow-up arranged by the care team. The plan may include ongoing exercise, medicines agreed with the clinician and regular reviews. WeGuide supports this next stage with patient education, reminders and scheduled check-ins, so relevant updates are ready for each review.
Contact WeGuide to discuss your cardiac rehabilitation program, patient population and existing workflow, whether centre-based, home-based or hybrid. Together, we can identify the intake and readiness forms, education modules, exercise log, check-in schedule and any integrations needed. Your clinical team defines the exercise prescription, safety assessment and all care decisions.
WeGuide pricing depends on the scope of your care pathway, configuration, integrations and support requirements. Contact our team to discuss your cardiac rehab program and request a proposal. Clinical assessment, supervised exercise and program fees are determined separately by the care provider.
Request patient access
Not all clinicians are using WeGuide yet. If yours isn't connected, share their practice details so we can discuss access to the cardiac rehabilitation pathway with your care team.
Requesting access does not book an appointment. Your cardiac rehab team decides which program and next steps are right for you.
