The Montreal Cognitive Assessment is a one page, 30 point screening test covering visuospatial and executive function, naming, memory, attention, language, abstraction and orientation. It was designed specifically to detect mild cognitive impairment, a range in which the Mini-Mental State Examination performs poorly.

What is the Montreal Cognitive Assessment

The Montreal Cognitive Assessment covers seven cognitive domains in about ten minutes: visuospatial and executive function through a trail making task, cube copy and clock drawing; naming; memory through delayed recall of five words; attention through digit span, vigilance and serial subtraction; language through repetition and verbal fluency; abstraction; and orientation.

It was developed to fill a specific gap. Brief global screens are reasonably sensitive to established dementia and insensitive to mild cognitive impairment, largely because they under-sample executive function and delayed recall. The MoCA weights both more heavily, which is why it detects impairment that shorter screens miss and why it is harder for people with intact cognition to score full marks.

MoCA Scoring

Scores run from zero to 30. A cut-off of 26 was proposed in the original validation, with one point added for people with 12 years of education or fewer. Later work has repeatedly found that a single cut-off does not transfer across populations: education, first language and cultural familiarity with the test material all shift scores, and applying an imported threshold will misclassify people.

Two practical points. The MoCA requires training and, since 2020, certification to administer, and use is controlled by the copyright holder. Multiple alternate forms exist and should be rotated in longitudinal studies, because repeating the same version produces a practice effect that can mask real decline.
