What is Hoehn and Yahr Scale

The Hoehn and Yahr Scale is a trusted tool for tracking Parkinson's disease progression. It helps clinicians define clinical stages and symptom severity. This scale captures the general patterns of progressive decline. It focuses heavily on motor symptoms and functional disability levels rather than issues unrelated to movement. Originally featuring five distinct stages, the modified version adds 0.5 increments for better precision. It ranges from stage one with symptoms on one side to stage five where a patient needs a wheelchair or is bedridden without help. Doctors use this simple rating system during routine exams to guide treatment decisions. It is quick to administer and requires no special equipment. This makes it practical for busy clinics and research trials where tracking changes over time is essential for managing ongoing patient care plans.

H&Y Scoring

The Hoehn and Yahr Scale can be scored using two main approaches: the classic staging system and the modified version. 1. Classic System: Clinicians assign a stage from 1 to 5 based on symptom severity. Stage 1 covers one sided symptoms, while Stage 5 means the patient is wheelchair bound or bedridden. It gives a quick snapshot of disability levels. 2. Modified Version: This update adds stages 1.5 and 2.5 to catch finer details in progression. It accounts for unilateral symptoms with axial involvement or mild bilateral disease with recovery on the pull test. This helps track the middle phases better.
