The STRATIFY Fall Risk Assessment Tool can be scored using one simple approach based on five clinical risk factors. You will look at areas like past falls, agitation, visual impairment, frequent toileting, and mobility transfer ability. For each risk factor present, you just give a score of 1. If the factor is not there, it is a 0. Add these up for a total between 0 and 5. Generally, a total score of 2 or more suggests the patient is at high risk of falling.
