The pitting oedema scale is a bedside grading of peripheral oedema. The examiner presses a thumb over a bony prominence for several seconds and grades the depth of the resulting pit and the time it takes to disappear, usually on a one to four scale.

What is the Pitting Oedema Scale

The examiner applies firm thumb pressure over a bony prominence, most often the medial malleolus, the dorsum of the foot or the tibia, holds it for around five seconds and observes the indentation left behind. Grading combines two observations: how deep the pit is, and how long it takes to rebound. The most common scheme runs 1+ for a slight pit that rebounds immediately through to 4+ for a deep pit that persists for several minutes.

It is worth being clear about what this scale is and is not. It is a structured description of a physical sign, not a validated psychometric instrument, and no single scheme is universally agreed. Published versions differ in both the depth thresholds and the rebound times they assign to each grade, so a grade recorded without naming the scheme is ambiguous.

Grades are recorded per site and per side rather than as a total. Record which anatomical site was tested, how long pressure was applied and which grading scheme was used, because these three details determine whether a grade recorded today is comparable with one recorded last week.

Where the change in oedema matters more than its category, limb circumference measured at marked points, or serial weight, are more sensitive and more reproducible than the pitting grade. Non-pitting oedema is a different sign with different causes and is not graded on this scale.
