The Borderline Symptom List (BSL-23) is a reliable tool for checking Borderline Personality Disorder severity. It helps clinicians track changes over time. This shortened version of the original BSL-95 focuses heavily on the specific distress patients feel. It cuts out the noise to look at subjective suffering clearly and accurately. Patients rate twenty-three items on a scale from zero to four based on how they felt over the last week. It covers key issues like emotional instability, feelings of emptiness, and urges to harm themselves without being too long or hard to finish for the client. Since it is quick to complete, this tool is great for repeated use during therapy sessions. It shows if treatment is working by spotting small shifts in symptom load. Clinicians value it because it is highly sensitive to change and offers a clear picture of a patient's current mental state and progress.
The Borderline Symptom List (BSL-23) can be scored using two main approaches: the Mean Score and the Sum Score. 1. Mean Score: You work this out by adding up all 23 items and dividing by the total number of questions. Since responses range from 0 for not at all to 4 for very strong, you get a final number between 0 and 4. Higher scores show much more severe distress. 2. Sum Score: This method simply totals the raw numbers from each answer to get a value between 0 and 92. Clinicians often use the mean score to track changes over time or compare against specific cut off points to help confirm a diagnosis.
