Pain Assessment in Impaired Cognition (PAIC)

The Pain Assessment in Impaired Cognition (PAIC) helps clinicians identify pain in patients unable to self-report, particularly those with dementia. By observing facial expressions, movements, and vocalisations, this tool ensures accurate pain management. It's essential for maintaining comfort in aged care settings.

What is Pain Assessment in Impaired Cognition (PAIC)

The Pain Assessment in Impaired Cognition (PAIC) helps clinicians spot pain in people who can't speak up. It acts as a key tool for better dementia care. This observational scale looks at specific behaviours that suggest distress. It blends items from existing tools to create a standard way to check for discomfort. The assessment breaks things down into three main areas which are facial expressions, body movements and vocal sounds. Carers rate items like frowning or groaning to see how intense the pain might be without needing the patient to say a single word. Nurses and doctors find it easy to use during daily rounds. It works well in aged care homes where residents often have trouble communicating needs. By using this reliable method, healthcare teams can manage meds better and ensure patients stay comfortable. It really helps improve daily quality of life.

PAIC Scoring

The PAIC 15 can be scored using one main approach focused on observing specific behaviours. You assess fifteen items across three categories like facial expressions, body movements, and vocal sounds. For every item, you rate the intensity on a four point scale from zero, meaning not at all, to three, meaning very strong. Once you have rated all the signs, simply add them up to get a total score between zero and forty five. Higher numbers suggest more intense pain, helping carers spot discomfort even when the patient cannot speak up.
