What is Voice Handicap Index (VHI)

The Voice Handicap Index (VHI) is a trusted tool for assessing how voice disorders impact a patient's daily quality of life and emotional health. This questionnaire with thirty items helps clinicians understand the patient's perspective. It looks past physical symptoms to see the real struggle of living with voice issues. The survey splits into three distinct domains that cover functional, physical, and emotional aspects. Patients rate statements on a scale from zero to four, allowing for a clear picture of the severity and specific areas of concern. This tool is easy to administer in clinical settings and works well for tracking progress over time. Speech pathologists use the total score to plan therapy and measure outcomes. Because it focuses on the patient's own view, it ensures treatment targets exactly what matters most to the individual.

VHI Scoring

The Voice Handicap Index can be scored using a standard approach that sums responses across three key domains. Patients rate 30 items from 0 for never to 4 for always. These items are split evenly into functional, physical, and emotional subscales. You sum the responses to get a total score between 0 and 120. A higher tally suggests a bigger impact on quality of life. Generally, scores under 30 show a mild issue, 31 to 60 indicate moderate problems, and anything over 60 points to a severe handicap. This helps speech pathologists track progress during therapy.
