The Social Phobia Inventory (SPIN) is a trusted self-rated scale used to measure social anxiety severity. It helps clinicians identify symptoms quickly. Developed by Connor and colleagues, this 17-item questionnaire specifically targets the three main symptom domains of social anxiety disorder found in various clinical settings. It evaluates fear of social situations, avoidance behaviours, and physiological arousal signs like sweating or blushing. Patients rate their problems over the past week on a simple scale from zero to four, ranging from not at all to extremely distressing. The SPIN is simple to score and takes only a few minutes to complete. A total score of 19 or higher usually suggests social phobia is present, making it a reliable screening option. It is excellent for tracking treatment progress, allowing clinicians to monitor how well therapy works in daily life.

The Social Phobia Inventory can be scored using one main approach: calculating the total symptom severity.1. Total Sum Score: You'll add up the responses from all 17 items, rated on a scale from 0 to 4. This creates a total ranging from 0 to 68. Scores are then grouped into severity levels, where anything under 20 is considered minimal, while scores over 50 show very severe social anxiety. A cutoff of 19 helps separate those with social phobia from those without. It's a solid way to measure fear, avoidance and physiological discomfort in clinical practice.
