Clinicians rate facial expressions, body movements, and muscle tension. They also check if the patient fights the ventilator or makes vocal sounds. Each area gets a score from zero to two, creating a total out of eight to show current pain levels. It is quick to use at the bedside and works well for adults with or without a breathing tube. Nurses use it to guide pain relief choices so patients stay comfortable. Regular checks with this scale help stop untreated pain and support better recovery outcomes in busy intensive care settings.

CPOT Scoring

The Critical Care Pain Observation Tool is scored using a single cumulative approach based on four behaviours. Clinicians assess facial expression, body movements, muscle tension, and ventilator compliance or vocalisation for extubated patients. Each category gets rated from 0 to 2 based on severity. You just add these up to get a total score ranging from 0 to 8. A total score greater than 2 generally suggests the patient is in significant pain. This cutoff helps nurses decide when to give analgesics or check for other issues.
