The Penetration-Aspiration Scale (PAS) is a trusted tool for measuring swallowing safety. It helps clinicians rate airway invasion depth accurately. This 8-point observation tool evaluates two main things. It looks at how deep material travels into the airway and whether the patient can clear it out effectively. Scores range from level one where nothing enters the airway to level eight where food passes below the vocal folds without any effort to eject. Clinicians watch for residue and the patient's sensory response to determine the severity of the aspiration risk. Speech pathologists use the PAS during videofluoroscopic swallow studies or fibreoptic evaluations. It provides a clear number for describing dysphagia severity which makes tracking changes over time much easier. It’s a practical way to guide diet changes and therapy planning for better safety.

PAS Scoring

The Penetration Aspiration Scale can be scored using a single 8 point ranking system based on airway safety. Clinicians assign a rating from 1 to 8 depending on how deep material goes into the airway and if the patient clears it. A score of 1 is normal, while scores 2 to 5 show penetration where stuff stays above the vocal cords. Scores 6 to 8 indicate aspiration, meaning food or liquid drops below the cords. Higher numbers signal severe dysphagia and often a lack of sensation. This helps speech pathologists track swallowing safety during swallow tests like videofluoroscopy.
