What is Neuropsychiatric Inventory (NPI)

The Neuropsychiatric Inventory (NPI) is a widely used tool for assessing dementia-related behaviours. It helps clinicians track changes over time. This assessment relies on a structured interview with a caregiver who knows the patient well. It looks at twelve distinct behavioural areas that are common in dementia cases. The tool covers symptoms like agitation, apathy, and hallucinations. For each area, the caregiver rates how often it happens and how severe it is. They also rate their own distress levels, which gives a full picture of the burden involved. It usually takes about ten to twenty minutes to finish, making it practical for busy clinics. Doctors use the scores to see if medications or therapies are working effectively. By spotting specific issues early, care teams can adjust their plans to support both the patient and the family better.

NPI Scoring

The Neuropsychiatric Inventory can be scored using two main approaches: Symptom Severity and Caregiver Distress. 1. Symptom Severity: For each behavioural domain, multiply the frequency (1 to 4) by the severity (1 to 3). This produces a domain score up to 12. Sum these across all 12 domains for a total between 0 and 144 to measure overall symptom load. 2. Caregiver Distress: Carers rate their emotional stress for each symptom from 0 (none) to 5 (extreme). This separate score helps teams identify carer burnout risks.
