The Delirium Rating Scale - Revised (DRS-R-98) is a widely used tool for assessing symptom severity and diagnosing delirium in clinical settings. This sixteen-item scale splits into a thirteen-item severity section and three diagnostic items, helping clinicians track changes over a twenty-four-hour period. It covers a broad range of symptoms, looking at cognitive issues like attention and orientation, plus non-cognitive traits like sleep-wake cycles and motor agitation. Each item gets scored from zero to three based on interview data and family reports. Ideally suited for psychiatrists and geriatric teams, it takes roughly ten minutes to complete. It’s sensitive for detecting symptom patterns in complex cases. By using multiple data sources, it ensures accurate diagnosis, making it a reliable choice for monitoring patient recovery in busy wards.

DRS-R-98 Scoring

The DRS-R-98 can be scored using two main approaches: the Total Score and the Severity Score. 1. Total Score: You sum all 16 items, rated 0 to 3, covering diagnostic features and symptom severity. Scores range from 0 to 46, with totals over 15 usually pointing to full delirium. 2. Severity Score: This method sums only items 4 to 16, skipping the first three diagnostic questions. It gives a range of 0 to 39 to track how bad symptoms are over time. It is really handy for checking if a treatment plan is actually working.
